Enfermedad tiroidea ocular y orbitaria

Thyroid eye disease refers to a group of conditions in which the tissues around the eyes become swollen, irritated, or pushed forward, most often in connection with thyroid problems such as Graves’ disease. In these conditions, the immune system mistakenly targets the muscles and fat behind the eyes, causing inflammation and changes in the eye socket (orbit). This can lead to bulging eyes, a “staring” appearance, and difficulty fully closing the eyelids, which may leave the eyes feeling dry, gritty, or uncomfortable.

Common symptoms include redness and irritation of the eyes, tearing, light sensitivity, and a feeling of pressure or fullness behind the eyes. Some patients report double vision, difficulty moving the eyes in certain directions, or pain with eye movements. In more advanced cases, the eyelids may retract (pull back), the eyes may appear misaligned, and at times, vision can become blurry or dim if swelling presses on the optic nerve. Symptoms often fluctuate and may not always match the thyroid disease’s activity on blood tests, making a specialized eye evaluation important.

Treatment focuses on relieving symptoms, protecting the eye’s surface, and, when needed, correcting changes in eye position or vision. Options may include lubricating eye drops and ointments, eyelid taping at night, and smoking cessation (which is especially important because smoking worsens thyroid eye disease). Depending on severity, patients may also benefit from anti-inflammatory medications, targeted infusions, or short courses of steroids, as well as specialized radiation or surgery to create more space in the orbit (orbital decompression), straighten the eye muscles, or adjust the eyelids. Multidisciplinary care is typically coordinated between an ophthalmologist and an endocrinologist, with regular monitoring to track changes over time and decide on the safest, most effective treatment plan.
