Information for Physicians
The Montefiore Einstein Command Center works with physicians to facilitate transfers to one of our clinical locations and is staffed by knowledgeable, experienced coordinators. Our team will match the patient with the most appropriate level of care and location.
Be prepared to answer healthcare and basic demographic information about patients including:
- Patient’s name and date of birth
- Referring physician’s name and number
- Current patient location and unit phone number
- Diagnosis and medical service(s) needed
- Special needs (isolation, ventilator, or IVs)
- Transportation Needs (ALS/BLS/Critical Care/Medical Air)
