Medical records can be requested by patients, authorized patient representatives, or legally permitted third parties (such as physicians, attorneys, courts, or insurance companies).

If you are an existing patient looking for everyday health information, you may not need to submit a formal records request. You can view your records 24/7 through Montefiore MyChart, our secure online patient portal.
Through MyChart, you can:

Montefiore Medical Center has online tools that allow patients, authorized representatives, and third parties to submit digital requests in a secure manner.

Special Circumstances:
- If the patient is incapacitated, or unable to sign for him/herself: the patient representative must enter their relationship to the patient so that we can determine authority to request the records.
- If the patient is deceased: the court-appointed administrator or executer of the state can request the medical records. A copy of the death certificate will be required to verify death if the patient did not expire in our facilities.
- Other circumstances additional documentation may be required depending on the situation. For questions, please contact 718-920-4921.
Delivery Expectations
In accordance with New York State law, requests are processed within ten days after the authorization is received.

La ley HIPAA le otorga el derecho a modificar su historial médico. Si considera que la información en su historial es incorrecta, puede solicitar una modificación. Un proveedor de atención médica revisará su solicitud y le responderá. Puede presentar una solicitud de modificación de historial médico en persona visitando una de nuestras centros médicos de Gestión de Información de Salud (HIM por sus siglas en inglés) de Montefiore, donde un miembro de nuestro equipo coordinará el proceso de modificación.
