Bronx Equity Integrated Care for Kids New York (BE-InCK NY)

What Is the InCK Model?

The Integrated Care for Kids (InCK) Model is a child-centered local service delivery and state payment model that aims to reduce expenditures and improve the quality of care for children under 21 years of age and pregnant individuals of all ages who are covered by Medicaid. The model will empower states and local providers to deliver better physical and behavioral healthcare through prevention, early identification and treatment. This project was awarded to the New York State Department of Health (NYS DOH) in 2020, and Montefiore Einstein was selected as the lead organization.

In collaboration with the NYS DOH, the name Bronx Equity-InCK NY (BE-InCK NY) was chosen to highlight our commitment to addressing equity issues to successfully improve the overall wellness and health outcomes in the Bronx community. Our model strives to improve access to care, support and services as well as the integration of primary and specialty care, while reducing avoidable inpatient stays and out-of-home placements. We will achieve this using a two-generation approach to care while applying a health equity lens to all the work we do.

This project is supported by the Centers for Medicare and Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS), as part of an award totaling $12,000,000 as of 2024, with 0% financed from nongovernmental sources. About Us

Who Do We Serve?

Individuals eligible for this model must be:

  • Medicaid beneficiaries
  • Under the age of 21 or pregnant, AND
  • Reside in service zip codes 10461, 10467 or 10469

How Do We Help?

A BE-InCK NY representative will contact you to explain the program and identify the services that may meet your child’s needs, including physical health, mental health, education, food, housing, other social service needs and care coordination.

For example:

  • If your child has multiple needs, they may be eligible for care coordination through BE-InCK NY or another program.
  • We can help identify someone who can help you coordinate the care your child is receiving from many providers to improve their health and wellness.
  • We can help you identify someone who can help you create a care plan to address the issues that affect your child’s health and quality of life.
  • We can help you identify someone who can connect your child and family with Mobile Crisis services and a crisis hotline for behavioral health emergencies.
  • We can help you identify someone who can help you access prenatal and postpartum care support services.

How Much Does It Cost?

There is no cost to participate.

BE-InCK NY receives a list of eligible individuals from the NYS Department of Health.

Involvement with BE-InCK NY does not change or affect your Medicaid status, immigration status or the services you can receive from your providers.

Note: BE-InCK NY is open to all Medicaid participants in these three zip codes: 10461, 10467 and 10469. It is NOT limited to Montefiore Einstein patients.
