The Clinical Diabetes Center's multidisciplinary team cares for adults with diabetes in both inpatient and outpatient settings. The diabetes center has more than 30 full-time faculty members and is the largest such center in the New York metropolitan area. We emphasize teaching patients how to improve their health through diet and medication and how to monitor their blood glucose levels and use insulin pumps.

Our Inpatient Program

We offer diabetes consultation for patients hospitalized at Montefiore Medical Center and are committed to communicating effectively with patients and providers. Our diabetes team identifies patients with diabetes upon admission and relays critical diabetes-related information to the primary care provider throughout each patient's hospital stay. This early, proactive approach has improved the blood-glucose control and well -being of our hospital patients.

Our Outpatient Program

Nuestra clínica ambulatoria atiende a adultos mayores de 18 años que son derivados por su médico de atención primaria para recibir educación y autocontrol. La educación sobre la diabetes que ofrecemos incluye asesoramiento nutricional, capacitación el uso de la bomba de insulina y monitoreo de la glucosa subcutánea. PROMISED© Program

Montefiore's diabetes self-management program, the Proactive Managed Information System for Education in Diabetes (PROMISED©) has been recognized by the American Diabetes Association for its exemplary performance and for consistently meeting national standards.

The PROMISED© Program provides 10 hours of education to patients with type 2 diabetes. The Center works closely with primary care physicians and has improved clinical outcomes in more than 1,000 patients. For more information about the program, please call 718-920-7247.

Support Groups

Our support groups meet regularly at Montefiore Hospital and at Jack D. Weiler Hospital to help type 1 and type 2 diabetes patients maintain health management skills and stabilize their conditions. For more information about support groups, please call 718-920-7247.
