Bronx Accountable Healthcare Network Health Home (BAHN)
The Bronx Accountable Healthcare Network, also known as the BAHN, is a Health Home program serving the Bronx. The BAHN Health Home is not a physical place; it is a group of Bronx healthcare and service providers that work together to make sure Bronx residents, both adults and children, receive the care and services they need to fulfill their wellness goals.
The BAHN Health Home is led by Montefiore Medical Center and supported by several other highly recognized Care Management Agencies—all leaders in community-based healthcare and social services--that you know and trust. Our experience and expertise in providing person-centered care to individuals and families in the community will help you and your family to obtain optimal health.
When you enroll in the BAHN Health Home, you will be connected to one of our Care Management Agencies and you will be assigned your own Care Manager to work directly with you to develop an individualized Plan of Care that maps the services you need to keep you on the road to better health.
Some of these services include:
- Connecting to health care providers
- Connecting to mental health and substance abuse providers
- Connecting to needed medications
- Assistance with housing
- Social services — such as food, benefits, and transportation
- Other community programs that can support and assist you
