Refer a Patient

At Montefiore Einstein Urology, we know that providing patients with the best possible care comes down to teamwork and trust. We routinely accept transfers from other facilities, and we work closely with our referring physicians to ensure open channels of communication and reliable tertiary expertise.

To help ensure a seamless referral and/or second opinion, please have the following information ready when contacting us:

• Patient’s name and contact information
• Patient’s date of birth
• Patient’s diagnosis
• Date of diagnosis
• How the diagnosis was made (i.e. exam, biopsy, other)
• Supporting medical imaging and reports
• Patient’s course of treatment and period administered (if any)
• Patient’s insurance information
• Referring physician’s office address and telephone/fax numbers
