Montefiore Einstein Command Center provides a 24-hour, one-call solution for physicians to transfer patients into the Montefiore Einstein’s Health System or move patients from within the system to a higher level of care.

Information for Physicians
The Montefiore Einstein Command Center works with physicians to facilitate transfers to one of our clinical locations and is staffed by knowledgable, experienced coordinators. Our team will match the patient with the most appropriate level of care and location.
Be prepared to answer healthcare and basic demographic information about patients including:
- Patient’s name and date of birth
- Referring physician’s name and number
- Current patient location and unit phone number
- Diagnosis and medical service(s) needed
- Special needs (isolation, ventilator, or IVs)
- Transportation Needs (ALS/BLS/Critical Care/Medical Air)
