Montefiore has developed and relied on advanced models of healthcare delivery and innovative payment methodologies since 1996. This leadership in coordinating care across multiple settings earned us recognition from the federal Centers for Medicare & Medicaid Innovation (CMMI). In 2011, the Montefiore Accountable Care Organization (ACO) was designated as one of the first Pioneer ACOs. The Pioneer ACO is one of a number of initiatives developed by CMMI to test new models of health care delivery and payment. The Montefiore ACO was chosen in a competitive process in which it demonstrated its experience in coordinating care for patients across multiple care settings.
