High-Risk Care Management

The High-Risk Care Management team at Montefiore Einstein provides intensive care management to patients following discharge from an inpatient psychiatric unit or after an emergency room visit. Our program is led by experienced clinical staff and includes a team of peer specialists who bring lived experience and a unique understanding to the care process. Together, we offer personalized care coordination and advocacy to promote mental wellness, improve health outcomes and guide patients through behavioral healthcare challenges.

The High-Risk Care Management team focuses on managing the complex medical needs of patients who are considered high-risk due to severe chronic conditions, advanced stages of disease or severe health complications.

To optimize patient outcomes, our team coordinates consultations with specialists, designs individualized comprehensive care plans, helps manage medications, coordinates appointments and facilitates communication between various healthcare providers.
