Montefiore Einstein Center for Heart and Vascular Care is committed to preventing heart disease. Our multidisciplinary team offers a comprehensive range of services aimed at improving patients’ heart health through assessment strategies and lifestyle changes. We offer a comprehensive risk assessment, including the use of imaging modalities, such as coronary calcium scoring (CAC) and coronary computed tomography angiography (CCTA), to provide a tailored baseline of heart health for each patient.  Our program’s range includes reducing risk factors in patients with heart disease to prevent further complications, including recurrent heart attacks or heart failure, to preventive care plans designed to protect healthy hearts. Personalized Management of Risk Factors

Our expert cardiologists tailor each treatment plan to meet a patient’s specific needs based on their medical history, risk factors and lifestyle. While heart disease is hereditary, each patient faces different preventable risk factors that contribute to the likelihood of developing the disease. These risk factors include:

Cardiac Wellness Program

The Montefiore Einstein Cardiac Wellness Program, which is part of the Preventive Cardiology and Cardio-Metabolic Program, brings a unique, nutrition-centered approach to the management of cardiovascular disease. The program aims to prevent and reverse heart disease by encouraging patients to embrace a diet with no animal products and consisting mainly of vegetables, fruits, whole grains, beans, legumes (e.g., lentils or chickpeas), a small amount of nuts and avocado, and dairy alternatives.

Adopting a Whole Food/Plant-Based Diet

For many patients, shifting to a whole food/plant-based diet can be daunting. Our Cardiac Wellness Program, led by nationally-renowned cardiologist Dr. Robert Ostfeld and expert nutritionist Lauren Graf, can help patients smooth the transition, develop a personalized approach to plant-based ingredients and recipes, and learn how a whole food/plant-based diet can prevent a range of diseases,  promote healthy weight and blood pressure, manage cholesterol and blood sugar levels, and aid digestion.

How to register:

  1. Schedule an initial consultation with our team of experts, during which you can share your health goals and concerns.
  2. Attend an educational session on implementing a plant-based diet and lifestyle. We welcome patients to bring a companion or caregiver to the session.
  3. Once you’re on your way, youll receive follow-up communications from our team to assess your progress and make adjustments as necessary.
