Sleep-Wake Disorders Fellowship Program

Overview

The Sleep-Wake Disorders Center at Montefiore Einstein was established in 1975 as a unique clinical and research facility. In 1977, we became the inaugural program accredited by the Association of Sleep Disorders Centers, now known as the American Academy of Sleep Medicine. Since its inception, our Sleep-Wake Disorders Center has cared for patients with a spectrum of disease entities, including sleep-related breathing disorders, parasomnias, insomnias, circadian rhythm disorders, and sleep disorders associated with medical and psychiatric disorders.

ACGME-accredited since 2005, our one-year Sleep Medicine Fellowship Program is devoted to ambulatory and inpatient clinical experiences. The program offers fellows the opportunity to rotate through three primary areas of sleep medicine at Montefiore Einstein: the adult neurology-focused Sleep-Wake Disorders Center, the pediatric Sleep Disorders Center at CHAM and a pulmonary-focused sleep-related breathing disorders facility. Additionally, fellows rotate with dentists performing oral appliance fittings for sleep apnea management and Ear Nose and Throat (ENT) specialists evaluating patients for surgical options for sleep apnea management.

Clinical Practice

Fellows will evaluate and care for patients with sleep disorders in inpatient and outpatient settings.

The inpatients are seen as consults. The sleep medicine consults to the Pulmonary service are primarily for medical (pulmonary, internal medicine, cardiology) and surgical patients. The consults called into the Neurology service are primarily for Neurology/Epilepsy and Pediatric patients.

The outpatients are seen in the subspecialty clinics. In the Adult Pulmonary Sleep Clinic the fellows see patients and are supervised by David Appel, MD, Diplomate of the ABSM. In the Adult Neurology sleep clinic, the fellows see patients and are supervised by Michael Thorpy, MD, Diplomate of the ABSM; Imran Ahmed, MD Diplomate of the AAN in Neurology and Sleep Medicine; Renee Monderer, MD, Diplomate of the AAN in Neurology and Sleep Medicine; Usman Riz, MD, Diplomate of the AAN in Psychiatry and Sleep Medicine; and Kristina Maselli, MD, Diplomate of the AAN in Neurology, Board eligible in Sleep Medicine.

At the Pediatric Pulmonary Sleep Clinic the fellows see their own patients as well as new patients and are supervised by Ranaan Arens, MD, Diplomate of the ABSM. In the ENT Clinic the fellows see new and follow up patients and are supervised by Marc Gibber, MD.

Fellows will also have formal education and clinical experience with performing and interpreting diagnostic tests, including: polysomnography, MSLT, MWT, CPAP/BPAP/ASV titration/use, PAP-NAPs home sleep studies as well as Actigraphy.
