Behavioral Health Integration Program (BHIP)
At Montefiore Einstein, the path to total health and wellness requires prioritizing the mental health of our communities. Our Behavioral Health Integration Program (BHIP) collaborates with your Montefiore Medical Group (MMG) primary care medical provider to support you holistically by coordinating treatment to meet your physical, mental and social health needs.
This holistic treatment goal is met by integrating BHIP into MMG’s primary care practices to provide short-term, evidence-based interventions that support the mental health of our patients and families across the lifespan. BHIP is the first program of its kind and serves as a model for other primary healthcare systems throughout the United States.
Our services are available through our primary care locations, allowing patients to access mental health interventions in a trusted and familiar setting. The program consists of two evidence-based models of care:
- Our team of HealthySteps specialists supports our younger patients and parents.
- For older children and adults, our Behavioral Health Collaborative Care Model (CoCM) provides behavioral health support for patients older than 5 years of age.
Our Services
To help us better understand every patient’s unique needs, BHIP provides a thorough medical screening that includes a mental health evaluation. All of our primary care locations have teams of physicians and specialists who work in partnership with a team of behavioral health practitioners, including social workers, psychologists and psychiatrists.
BHIP’s mental health services include mental health care coordination focused on supporting and treating:

HealthySteps
HealthySteps is a first-of-its-kind, nationally recognized preventative program that cares for families through the initial five years of a child’s development. This program ensures parents and their babies get started on the right track. At MMG, we understand that parenting is challenging despite the many expected joys, and babies do not come with operating manuals. HealthySteps integrates a behavior and development specialist and a social worker or a psychologist into pediatric practices so families do not need to rely solely on parenting books, blogs and podcasts for advice and guidance.
The HealthySteps program supports early childhood development in several ways. First, we administer universal screenings to monitor young children’s development and social-emotional functioning during the first three years of life. When medical providers or parents have concerns or questions about their child’s development or behavior, families can meet with a HealthySteps specialist.
HealthySteps providers help with many common parenting concerns, such as temper tantrums, picky eating or developmental delays, and provide support for family concerns such as postpartum depression. Patients can meet with their HealthySteps specialists either virtually or in person. Because HealthySteps providers are integrated into medical practices, parents can often see their HealthySteps provider during their regular well-child visits.
Collaborative Care Model (CoCM)
The BHIP Collaborative Care team at MMG works alongside your medical provider to provide behavioral healthcare management, short-term psychotherapy and psychiatric consultation for children aged five and older, adolescents and adults. Our treatment teams are staffed with social workers, psychologists, community health workers, care managers and psychiatrists who collaborate with medical staff to improve outcomes and the quality of life for our patients and their families.
The Collaborative Care Model (CoCM) is an approach to mental health treatment based on core principles for effectively managing chronic illnesses. Referred and enrolled patients will be followed by an interdisciplinary team consisting of your medical doctor, a care manager, a behavioral health provider and a consulting psychiatrist. Your treatment team will monitor your symptoms closely using validated rating scales, which will help guide your treatment and your team’s clinical decision-making.
Our BHIP CoCM team provides access to evaluation and treatment for all referred patients. While some patients may be able to receive their behavioral health services on-site, other patients may benefit more from treatment in the community. When patients are referred for illness requiring more intensive support and/or intervention, the BHIP CoCM team will help connect them to the right level of care based on their needs. This evidence-based and holistic approach leads to improved mental and physical health outcomes, allowing our patients to achieve their health and wellness goals.
