Our approach to senior care at Montefiore Medical Group (MMG) focuses on providing comprehensive and specialized healthcare services through primary care physicians to meet the unique needs of older adults. Our primary care providers work closely with a multidisciplinary team of experts who collaborate to proactively address these issues for seniors.
MMG senior care services are available through primary care providers to promote healthy aging, maximize independence and enhance the overall quality of life for older adults. We provide holistic, patient-centered and compassionate care during this stage of life, beginning with a thorough assessment, which includes cognitive screenings to detect early signs of cognitive impairment or dementia.
Our Services
The MMG Senior Care Department provides the following services to our community:

- Advanced Directives and goals of care discussions
- Connection to community resources for seniors
