Rachel B. R. Chalmer, MD, is an attending physician at Montefiore Einstein and Associate Professor, Geriatrics at our Albert Einstein College of Medicine. Dr. Chalmer primarily provides consultative care, including diagnosis and treatment, for individuals living with cognitive impairment or dementia and their families or care partners.

After obtaining her Bachelor of Arts and Sciences in Spanish, public health, pre-medical studies and medical humanities at the University of South Carolina in 2004, Dr. Chalmer earned her Doctor of Medicine at the Medical University of South Carolina in 2008. She completed her internship and residency in primary care and social internal medicine at Montefiore Einstein in 2011, followed by a fellowship in geriatric medicine at the same institution in 2017.

Dr. Chalmer's research interests include enhancing early detection and diagnosis of cognitive disorders, eliminating health disparities in these disorders, and improving dementia care in primary care. She has been project director and co-investigator on several funded research projects and has shared her work through numerous invited presentations and peer-reviewed journals.

Dr. Chalmer is board certified by the American Board of Internal Medicine in Internal Medicine and Geriatrics. She is a member of the Gold Humanism Honor Society, the American College of Physicians and the American Geriatrics Society. Dr. Chalmer has been recognized for her achievements in teaching with a membership in the Leo M. Davidoff Society. She was also a recipient of the Leo M. Davidoff Society Award from Montefiore Einstein for Outstanding Achievement in the Teaching of Medical Students. In 2019, she received the Montefiore Einstein Medical Student Teaching Star Program Commendation Letter. Dr. Chalmer is fluent in English and Spanish.
