What Are Ejaculatory Disorders?

Ejaculatory disorders are conditions that impact the process of male ejaculation, (the release of semen from the penis during sexual activity.) Ejaculatory disorders, also known as ejaculatory dysfunction, are the most common form of male sexual dysfunction and a leading cause of infertility in men. There are multiple types of ejaculatory disorders, each with unique causes and symptoms.

Types of Ejaculatory Disorders

  • Anejaculation: the inability to ejaculate during masturbation or sexual intercourse. Anejaculation can be situational or total. Situational anejaculation is often related to stress or other psychological factors, while total anejaculation can result from side effects of certain prescription medications and co-existing hormonal or physical conditions.
  • Delayed ejaculation: when ejaculation cannot occur after a sufficient amount of sexual stimulation. Some men who suffer from delayed ejaculation can ejaculate during masturbation but not during sexual intercourse. Symptoms can be lifelong or acquired, universal or limited to specific situations. Certain conditions including high blood pressure or the use of certain prescription medications can contribute to delayed ejaculation.
  • Painful ejaculation (dysorgasmia): pain or discomfort during or after ejaculation, caused by inflammation, infection, urinary obstructions and in rare cases, prostate cancer. Patients experiencing painful ejaculations should seek medical attention to determine its underlying cause.
  • Premature ejaculation (PE): occurs when a man ejaculates earlier than intended during sexual intercourse. PE is among the most common sexual conditions. According to the National Institutes of Health, approximately one in three men aged 18 to 29 experience premature ejaculation.
  • Retrograde ejaculation: occurs when semen enters the bladder instead of passing through the penis during climax. Retrograde ejaculation can lead to male infertility.

Causes of Ejaculatory Disorders

Ejaculatory disorders can be the result of a variety of psychological and biological causes. Physical factors including hormonal imbalances, prostate conditions, and side effects of prescription medication may contribute to ejaculatory disorders. Psychological factors including stress and anxiety may also be a contributing factor.

Diagnosing Ejaculatory Disorders

To diagnose ejaculatory disorders, your healthcare provider will conduct a physical exam and discuss your sexual history. A genital exam is also indicated in most cases. Blood tests, to evaluate testosterone levels, may also be ordered.

Treatment for Ejaculatory Disorders
