What Are Foreskin Problems?

Foreskin problems are characterized by swelling, tightness, dryness, irritation and infection of the skin covering the head of the penis in uncircumcised males. There are many types of foreskin problems, including phimosis, the inability to pull back the foreskin, and balanitis, a common infection at the head of the penis.

Causes of Foreskin Problems

Foreskin problems have a variety of causes, including:

  • Allergic reactions to chemicals or prescription medications
  • Balanitis (swelling or soreness of the tip of the penis), often linked to bacterial or fungal infection
  • Diabetes and other conditions
  • Poor hygiene of the foreskin and penis, which can lead to the growth of bacteria and fungus
  • Skin conditions, including eczema, psoriasis, penile adhesions or scar tissue

Signs & Symptoms of Foreskin Problems

Symptoms of foreskin problems include:

  • Difficulty urinating
  • Dryness, irritation, itchiness or rash
  • Frequent urinary tract infections (UTIs) 
  • Soreness or swelling
  • Split or bleeding skin

Two common conditions affecting the foreskin are phimosis and balanitis.

Phimosis
Tight foreskin at birth is common, and the skin loosens over time. However, some men continue to have problems with tight foreskin as they age. They may experience symptoms including:

  • Inability to pull back the foreskin and expose the head of the penis
  • Pain during sexual intercourse
  • Painful urination

Balanitis
In balanitis, inflammation of the glans (head of the penis) is typically accompanied by:

  • Penile pain
  • Shiny, whitish patches at the tip of the glans
  • Swelling of the penis

Diagnosing Foreskin Problems

Foreskin problems are diagnosed with a complete physical examination and a review of your medical history.

Treating Foreskin Problems

Foreskin problems caused by bacterial or fungal infection may be treated with moisturizing lotions, steroid creams, topical antibiotics, topical antifungals or oral medications. Improved hygiene practices to keep the penile area clean and germ-free may act as a preventative measure. For some men, surgery to loosen or completely remove the foreskin may be recommended to relieve symptoms and prevent recurrence.
