Ultrasound

Our mission is to train physicians to proficiently integrate point-of-care ultrasound (POCUS) into patient evaluation and management. In addition to resident training, Health + Hospitals | Jacobi offers a one-year Emergency Ultrasound Fellowship Program.

Emergency medicine (EM) residents at Jacobi Medical Center gain extensive POCUS experience. As a high-volume Level 1 trauma center, Jacobi exposes residents to diverse clinical pathologies and emergency ultrasound applications, including trauma, abdominal and cardiac emergencies, vascular access and ocular pathology. Montefiore Hospital, Moses Campus, and Jack D. Weiler Hospital, Einstein Campus, both tertiary academic centers, provide exposure to complex, high-acuity patients requiring intricate care.

During their junior years, EM residents complete two two-week ultrasound rotations. These rotations offer a comprehensive understanding of EM ultrasound through didactics, supervised scanning and scan review. Residents also gain significant obstetrics and gynecology (OB/GYN) ultrasound experience during two dedicated OB/GYN blocks in the Emergency Department (ED). Hands-on experience is further enhanced by ultrasound-guided simulation models at our Simulation Center. Many senior residents opt for elective time to refine their skills and learn advanced emergency ultrasound applications.

Beyond our ultrasound fellowship-trained attendings, Jacobi also has between one and three emergency ultrasound fellows who provide hands-on bedside teaching.

Simulation & Education Program

At the Jacobi Simulation Division, we strive to cultivate clinical excellence through a psychologically safe educational environment, prioritizing patient safety. Our Program aims to develop physicians proficient in educational theories, curriculum development, simulations and debriefing techniques.

Emergency medicine residents at Jacobi and Montefiore Einstein benefit from extensive simulation exposure, including monthly Grand Rounds with participants from all years and weekly postgraduate year (PGY)-specific simulations. This exciting addition to their robust clinical experience enables residents to practice skills in a secure and collaborative setting.

Residents also participate in a dedicated Education block, where they gain experience in developing cases, running scenarios under faculty guidance and structuring debriefs and receive targeted feedback on their educational skills.

In addition to accessing the NYC Health + Hospitals Simulation Center, our fellowship-trained simulation faculty conduct in situ simulations and procedural labs within the clinical work environment. These sessions are designed to identify latent safety threats and provide just-in-time training, allowing emergency medicine residents and physician assistants to build muscle memory in their actual clinical environment at both Campuses.

As a high-volume Level 1 trauma center, Jacobi Medical Center fosters monthly collaborative simulations between emergency medicine residents and trauma residents. This initiative aims to develop a high-functioning, collaborative team for every trauma notification in the Emergency Department.

Ultimately, our fellows are prepared to become leaders in the emergency medicine simulation community, facilitating resident education, research projects and integrating simulation into hospital-wide quality improvement initiatives.

Community Engagement

Jacobi and Montefiore Einstein are proud to be home to outreach programs that educate and improve our communities in the Bronx and beyond. Our residents are heavily involved in educating and helping patients and families through these programs.

Mentoring in Medicine

Founded and led by our own Dr. Lynne Holden, Professor of Emergency Medicine at Montefiore Einstein and a graduate of the Class of 1995, Mentoring in Medicine is a non-profit organization that introduces, educates and empowers disadvantaged and low-income students to pursue careers in medicine. Over a dozen of our current residents were past participants in this Program, whether during elementary, middle or high school. Some of them have said that without Mentoring in Medicine, they would have never discovered a career in emergency medicine. Through her team’s hard work, the Program is now active nationwide from our home in the Bronx all the way up to Alaska!

Stand Up to Violence

As a Level 1 Trauma Center, Jacobi treats a very high volume of patients who are victims of violent traumas. Stand Up to Violence was founded as a hospital-based violence prevention program to reduce rates of retaliation, provide resources to victims and their families and improve health outcomes in our community. The current Medical Director is Dr. Noé Romo, a pediatric hospitalist at Jacobi Medical Center and a Class of 2012 graduate of its Pediatrics Residency Program.

Hyperbaric Medicine

Located at Jacobi Medical Center, a New York City Level 1 Trauma Center and Burn Unit, our Hyperbaric Program offers state-of-the-art care. Our hyperbaric physicians collaborate closely with Jacobi’s burn specialists to treat victims of smoke inhalation injuries and provide wound care.

Since 1983, Jacobi Medical Center has treated over 3,000 patients with carbon monoxide poisoning, more than any other hospital in the United States. As the New York City referral center for the Divers Alert Network, Jacobi provides 24/7 emergency care for scuba divers with decompression sickness.

Jacobi Medical Center boasts the tristate area’s (New York, New Jersey and Connecticut) only walk-in hyperbaric chamber, which can accommodate up to nine patients simultaneously.

Our hyperbaric physicians are all board certified in emergency medicine and have undergone specialized training in hyperbaric medicine. They hold academic appointments at Albert Einstein College of Medicine in New York and are leaders in clinical research related to hyperbaric medicine.

Research

The Department of Emergency Medicine has over 30 years of experience investigating critical topics within the field. Our researchers have explored a wide array of subjects, including opiate and parenteral adjuncts in pain management, asthma management in the ED, human immunodeficiency virus (HIV) screening, COVID-19 best practices and advancements in resident and community education, among many others. Our research portfolio is as extensive and diverse as emergency medicine itself.

In recent years, we’ve expanded our mission to develop innovative solutions for current challenges in emergency medicine. Through interdepartmental and global startup collaborations, we remain at the forefront of ED-based biodesign and innovation. Current projects in this area include mHealth applications for improving care transitions in chronic disease patients, as well as remote monitoring and wearable health technology.

The Research Fellowship

The Montefiore Einstein Department of Emergency Medicine offers an EM Fellowship in Clinical Research. This two-year Program combines a didactic curriculum, leading to a Master of Science degree in clinical research methods, with mentored clinical research experience.

The Department provides comprehensive support for research faculty and projects, including 24-hour in-house research associates, statistical support and dedicated mentorship. Researchers also have access to the Montefiore Einstein Institute for Clinical and Translational Research (ICTR) through Albert Einstein College of Medicine. The ICTR’s core services include: 1) Clinical Trials and Regulatory Support Office; 2) Clinical Investigation Services Cores; 3) Biostatistics, Epidemiology and Research Design; 4) Biomarker and Analytic Research Core; 5) Biorepository; 6) Community Engagement Consultation Core; 7) Research Informatics Core, and 8) Project Acceleration Resource.
