Medical records can be requested by patients, authorized patient representatives, or legally permitted third parties (such as physicians, attorneys, courts, or insurance companies).
What to Know
Access Records Instantly via MyChart
If you are an existing patient looking for everyday health information, you may not need to submit a formal records request. You can view your records 24/7 through Montefiore MyChart, our secure online patient portal.
Through MyChart, you can:
• View lab and diagnostic results.
• Message your primary care or specialist doctors.
• Request medication refills.
• Manage your personal health history.

Request Medical Records Online
Montefiore Medical Center has online tools that allow patients, authorized representatives, and third parties to submit digital requests in a secure manner.
Recommended Browsers: Chrome, Safari and Firefox.
Use this tool if you are a patient requesting your own records, a parent requesting a minor child's records, or a caregiver with legal Power of Attorney.
- Note: The system will verify your identity by asking you to take a photo of your driver's license or ID using your smartphone or webcam. There is no fee for this service.

Special Circumstances:
- If the patient is incapacitated, or unable to sign for him/herself: the patient representative must enter their relationship to the patient so that we can determine authority to request the records.
- If the patient is deceased: the court-appointed administrator or executer of the state can request the medical records. A copy of the death certificate will be required to verify death if the patient did not expire in our facilities.

Delivery Expectations
In accordance with New York State law, requests are processed within ten days after the authorization is received.
Amendments
HIPAA provides you the right to amend your medical record. If you believe the information in your record is incorrect, you can request an amendment. A healthcare provider will review and respond to your request. A request to amend records can be filed in person by visiting one of our Montefiore Health Information Management (HIM) locations, where a member of our team can coordinate the amendment process.
