Checklist for Appointments:
Please have your pediatric endocrinologist or pediatrician fill out a transition summary, which needs to be sent to us prior to your scheduled visit. Please have your pediatric provider fax the form to our office prior to scheduling an appointment with Dr. Shivani Agarwal 844-556-6683 X 32
Bring the glucometers that you use
Bring your continuous glucose monitors and/or insulin pumps
Bring any recent blood work you may have done at your primary care visit or pediatric endocrinologist.
Please review your medications
Be prepared to declare your pharmacy of choice and the quantity and type of medications you will need.
