Clinical Diabetes Center
The Clinical Diabetes Center at Montefiore provides comprehensive care for adults with diabetes. Our multidisciplinary team of experienced endocrinologists, nurse practitioners, clinical nurse specialists and nutritionists provide diabetes management to 4,000 individuals annually in both inpatient and outpatient settings. We are committed to helping people with diabetes live full, healthy lives.
Nationally Recognized Diabetes Management Program
Montefiore's unique diabetes self-management program, the Proactive Managed Information System for Education in Diabetes (PROMISED©) was twice nationally recognized by the American Diabetes Association for exemplary performance and consistently meeting national standards.
Education and Access to Expert Care
The clinic serves adults age 18 and older who are referred by their primary care physician for education and self-management. We provide diabetes education including nutrition counseling, insulin pump training and subcutaneous glucose monitoring. Our goal is to teach and empower patients to improve their health through better management of diet and medication. We offer diabetes consultation for patients hospitalized at Montefiore and are committed to effective communication with patients and providers.
Individual appointments are available with our diabetes specialists throughout Montefiore's network of healthcare providers. Our experts are all faculty of Albert Einstein College of Medicine and are at the forefront of diabetes research and treatment.
National Institutes of Health and Other Funded Research
The Clinical Diabetes Center at Montefiore works in collaboration with the Einstein Diabetes Research Center (DRC). As a major site for National Institutes of Health (NIH)-funded basic and clinical research, the Center has participated in several landmark studies including the Diabetes Control and Complications Trial, proving that good control prevents complications in diabetes. Our partnership ensures that our diabetes care is based on cutting-edge research and innovative approaches to treatment.
Long-Term Support and Health Maintenance
Our support groups meet regularly at Montefiore Hospital and at Jack D. Weiler Hospital to help patients maintain lasting health management skills and stabilize their chronic conditions. For more information about support groups, please call 718-920-7247.
The PROMISED© Program provides 10 hours of patient education to patients with type 2 diabetes. The Center works closely with primary care physicians and has been shown to improve clinical outcomes in more than 1,000 patients. For more information about the program, please call 718-920-7247.
We are committed to serving our immediate community, which includes a large Hispanic population, and offer educational classes in English and Spanish. We work with The National Alliance for Hispanic Health and also have ongoing partnership with the Hispanic Center of Excellence at Albert Einstein College of Medicine, the first center of its kind in New York State.
