For Providers

The BAHN helps providers to care for our Health Home members by:

• Connecting members to appropriate care and community referrals
• Helping members maintain their health and access to care
• Helping members adhere to appointments, medication regiments and treatment plans
• Transitioning members from the hospital, or any institution, to the community
• Sharing communication of a single care plan for all service providers in the member’s care to address
  needs in a comprehensive manner
• Focusing on reducing gaps in care, including reinforcing member and provider goals, to help improve
  health outcomes
• Identifying barriers are social determinants of health that impact access to care
