Montefiore Nurse-Family Partnership
Montefiore’s nurse home-visiting service has offered community-based healthcare and other vital services where patients need it most—often beyond hospital walls—since the 1970s.
Now, as part of a collaboration designed to ensure that new mothers and their children have access to care that otherwise would be out of reach, we have joined forces with the New York City Department of Health’s Nurse-Family Partnership (NFP).
We selected the NFP as our partner based on its record of health outcomes and overall improvements in maternal health, child health and economic security for program participants.
About the Nurse-Family Partnership
Our new program provides home visiting services to Medicaid and WIC-eligible, first-time mothers with the goal of supporting and empowering both of them. How it works: Montefiore registered nurses visit your home on a weekly to biweekly basis, during your pregnancy and then afterward until your child reaches 2 years of age.
During home visits, our nurses will:

Monitor both your health and your baby’s health

Focus on how to encourage positive parent-child interaction

Offer information on social and community resources to help maximize awareness of community resources

Help identify ways to attain economic self-sufficiency

Create both long and short-term goals
To be eligible to participate in the Montefiore Nurse-Family Partnership, you must be:

A first-time mother with no previous live births
-
Low-income
-
No later than the 28th week of pregnancy
