Montefiore Einstein has partnered with Cadence to bring you the Montefiore Einstein Connected Care program, helping you to manage your health in between visits to your doctor.
What is Connected Care?
Remote patient monitoring and chronic care management help you stay connected with your Montefiore Einstein doctor. Using simple devices, you can measure important health information at home. Your care team checks these measurements to:
● Better manage ongoing health conditions
● Help you feel better day-to-day
● Catch health changes early
● Reduce trips to the hospital
Who Can Benefit from Connected Care?
This program is designed for patients with high blood pressure, as well as their families and caregivers. The Connected Care team will offer support in achieving patient blood pressure goals in between office visits.
How It Works
1. Getting Started
Your Montefiore Einstein doctor will recommend the program for you if they think it could have a positive impact on your health. You will then be enrolled in the program, which is powered by Cadence, and you'll receive an easy-to-use monitoring device at no extra cost. The device does not require any Wi-Fi or Bluetooth to operate.
2. Daily Checks
Take quick vitals readings using a blood pressure monitor from the comfort of your home as directed by your care team.
3. Automatic Updates
Your readings are automatically and securely sent to your care team without you having to do anything extra.
4. Personal Support
The care team will check in with you regularly during telephone visits. If any of your readings indicate a potential concern, the Care Team will contact you right away to help.
Common Questions
Does insurance cover this?
Yes. Remote patient monitoring is a covered benefit under many insurance plans and, after deductibles are met, the majority of people experience little to no cost for the program. The team will review your health plan coverage during your enrollment call.
What device will I receive?
Patients with hypertension will receive a blood pressure monitor.
How do I sign up?
Talk to your doctor to request a referral for the Montefiore Einstein Connected Care program. If your doctor recommends this program, the Cadence care team will reach out to you.
Is my health information safe?
Yes. All of your health information is protected and secure.
