Our Approach to Rare Disease Patients with Special Needs
At the New York Center for Rare Diseases, we are dedicated to helping families navigate the unique challenges of caring for individuals with rare diseases and making the highest-quality care more accessible to all patients. Our multidisciplinary team of world-renowned experts specializes in managing the diverse and complex needs of each patient and their families to help improve the quality of their lives. As a bridge between our institution and local communities, we offer access to numerous programs and services, integrating care and supporting rare disease patients and their families both inside and outside the walls of Montefiore Einstein.
Through our unique Linking Individual Needs of Children with Services (LINCS) Complex Care Program, we provide comprehensive and coordinated patient- and family-centered care for individuals with rare diseases. We support patients who live at home with medical complexity, have special needs and require complex medical care, including medical equipment such as breathing machines, feeding tubes and wheelchairs. We collaborate closely with our internationally-recognized pediatric and surgical specialists, outside home nursing agencies, regional and community pharmacies, local programs that provide non-medical home services, and home hospice programs to help integrate and coordinate care, guide management and ease the burden of care for patients and families.
Advanced Treatments & Technologies
We offer access to the most advanced treatments and technologies to help manage children and adults with rare conditions and special needs. Through our LINCS Complex Care Program, our multidisciplinary team of experts delivers the latest evidence-based longitudinal care services and integrated subspecialty care coordination. We use a modern, patient-centered medical home model for individuals with medical complexity.
For patients with feeding, breathing and/or functional limitations, we offer medical technology evaluations and access to the latest adaptive devices, including advanced prosthetics, robotics, exoskeletons, functional neuromuscular stimulation, 3D printing and the newest wheelchair technologies to enhance mobility and maximize function and quality of life. We also coordinate access to advanced ventilation systems, respiratory therapy devices, enteral nutrition and remote monitoring systems.
