Our Community Health Needs Assessments and Community Service Plans demonstrate our commitment to investing in disease prevention, health promotion and healthcare equity advancements. Regular reassessment of our communities’ needs and assets guides us in developing comprehensive strategies for addressing them effectively.

To better identify our communities’ significant health concerns, our assessment process is highly collaborative and includes input from residents, hospitals and healthcare providers, community organizations and partners like the New York City Department of Health and Mental Hygiene (DOHMH).

Our work is grounded in acknowledging and addressing the diverse age, ethnic, racial and cultural backgrounds within our communities, recognizing the historical disparities and inequities that significantly impact health outcomes. We serve counties with distinct characteristics—the Bronx, for example, is marked by New York City's youngest population, a wide range of languages spoken at home and a relatively smaller non-Hispanic white population, while Westchester boasts one of the highest median incomes and ranks as one of the healthiest counties in the state. These variations influence our data collection and inform the design of our initiatives.

Location also plays an important role in the creation of our plans. Despite improvements in health status in the Bronx, a persistent gap remains compared to other boroughs, with the Bronx being a hot spot for issues such as excess mortality, diabetes, obesity, asthma, drug/opioid use and HIV/AIDS in New York City.

Our major priorities and programs align with New York State’s Prevention Agenda, a statewide blueprint aimed at improving the health and well-being of all residents. By leveraging Montefiore Einstein's expertise in both clinical settings and the broader community, we consistently make a meaningful impact in the Bronx, particularly in the areas of prevention of communicable diseases and the promotion of health for women, infants, and children.

NYC Community Health Profiles

The New York City Department of Health and Mental Hygiene’s (DOHMH) Community Health Profiles cover 59 districts across New York City and provide over 50 metrics that focus on neighborhood-specific health conditions, from the prevalence of diabetes among adults to the number of bike lanes in your area. The profiles serve as more than just informative tools; they're a catalyst for meaningful action.

This comprehensive data equips our teams at Montefiore Einstein with valuable insights. By understanding the broader context of environmental factors and health outcomes, we can effectively plan and implement programs that target specific community needs, thereby advocating for positive change and significant investment in the health and vitality of our communities.
