Geriatrics
The Division of Geriatrics at Montefiore offers outstanding primary and consultative care for older adults. Our multidisciplinary team includes board-certified geriatricians, as well as nurse practitioners and social workers, who offer specialized diagnoses and treatments for the many chronic and acute health conditions facing older adults. We provide care in all settings, from the home to the hospital. Our dedicated physicians are all faculty members of Albert Einstein College of Medicine and strive to use the latest innovations in clinical practice.
Clinical Programs
Harry and Jeanette Weinberg Geriatrics Ambulatory Practice
The Harry and Jeanette Weinberg Geriatrics Ambulatory Practice offers primary care and consultations by a multidisciplinary team with expertise in the care of older adults.
Our services include:
- Dementia care
- Falls assessment
- Osteoporosis evaluation and other services in our Comprehensive Bone Center
Robert and Joyce Menschel Geriatric Home Visiting Program
Primary care and consultations in the patient's residence for homebound older adults.
Geriatrics Inpatient Service at Montefiore
Primary care and consultations are provided for older adults admitted to Montefiore at the patient's or physician's request. Our staff provide multidisciplinary care and address all aspects of a patient's care, including:
- Maintaining physical and cognitive function in the hospital
- Addressing decision making and advance directives
- Developing a discharge plan with the patient, family and caregivers
